SLIPPAGE OF HORIZONTAL TENDON STRANDS
The subject inquiry report is forwarded f' r infermation.

Based on discussion with licensee personnel, appropriate engineering effort
appears to have been initiated to determine the cause of the strand slippage
,
in tendon H-50 and its potential significance to the security of other
tendon anchors. This engineering effort includes investigation of wedges
and installation technique for short tendons.
We plan to further ewamine this matter during our next inspection in
September.
