Contrary to paragraph 4.4.1.3 of the Technical Specifications,.
which requires remotely operated R.1ctor Building isolation valves to be stroked every three months, one group of seven valves covered by Surveillance Procedure No. 205.07B was not stroked between October 3,1976 and April 6,1977.
B.
Contrary to paragraph 6.9.1 of the Technical Specifications and paragraph B.2.b(4) of Regulatory Guide 1.16, a cracked decay heat system socket weld discovered by the licensee on October 28, 1977, was not reported in writing to the NRC within 30 days.
