Dock 7.T h0. 50-.332 On October 24-25, 1973 I accompanied W. B. Swan on an inspection to the Cryotal River Unit No. 3 facility.
Zecause of the similarity in ' design i
of the cantuinnenta of Crystal River nud Thren Mila Island, I. ras requestad to review the con,truction procedures and the QA program for the constructico of the ring girdar. The objectiva of the inspection i
was to confirm that appropriata construction practices were used to prevent the for:::stion of voids in the concreta in crucial areas of the ring girder.
i.e., behind the tsaring plates and in other arcos cc,ngested by a largo nm bcr of retnforcing bara.
The ring girder in 17'-6" high end is being pourad in six lifts and in sl:ty de.;ree ac;unents. At the time of the inspection. the three bottoa lifts had2 been poured (the botton ten feet).
Preparation for the fourth: lift t.as undurway during cbc inspeccion.
To incilicata the placing of concrete in the constricted spacma of the ring girrier, a special coverute nix va s h. sing used.
The maxirnus.c.ggregata si:o van lic.ited to 1/" inch c.c.d the concreto slu=p to four inchec.
Pla::1glaaJ windowa worn incerted in the staal foms at the lccation of the bearing plates to allow the Quality Control (QC) people to observe the pinca:nent of the concreto in critieni areas.
The Quality Assurance (QA) records for the first three lifts were audited and the following items were noted as being outstanMng:

The specifications requires ' that epoxy be placed on the surface
' 'of the older concrete before a lift of fresh. concrete is placed.

The required 28 day strength of the concrete is 5000 psi Jand the cube strength of the epoxy is required to be 8000 psi. The
-QA records reveal that it was difficult to obtain the 8000pei specified in the specifications. _Most of the test results were in the range of 7500 psi.

We were informed that an engineering disposition ~of this problem had been nade by Gilbert Associates. The disposition was repertad to sey that the epoxy was not required to be any stronger than che concreta. Supporting doctmentation will be available for the inspector to review at the next regular inspection of this facility.
2.
It was found that NCR 511 dated September 18, 1973 has not been closed out.
It was claizaed by the QA personnal at the sita that an engineering disposition had been made but that the doctmeentation had not yet been received. This itein will be reviewed at the naxt regular inspection of this facility.
If the care and control e:rercisad by tha licensee in the pouring of tha first three lifts of the ring girder continues no construction anomalies should be experienced.
