lluman Factors Evaluation of Control Room Design and Operator Performance at TMI-2 On Monday, July 23, Gordon Chipman distributed the referenced document.
lie edited my draft of Iluman Factors issues which was distributed on Friday, July 20.
It was my thought that we would get together to add to or subtract from the list of issues included in my draf t and then assign the items to various people to assure that we will have a documented background for each for our final report.
Dennis Allison pointed out that the list of issues had dropped out
" changes made in control room after the accident." This item may be of little use, but it was dropped inadvertently.
Also not included in the list is the duty time of the TMI-2 operators, the shif t schedule, back-ground of the operators, and how the operators worked as a team.
There may well be other issues which shauld be added or some could be deleted.
It is, therefore, recommended that a meeting be held to review the referenced document for additions and deletions and assignment of tasks to individuals.
