This telegram is to confir:n the telecon between Nr. B. Davis (Region I-EO),

and J.G. Herbein (IMI Station Superintendent) at 1509 hours0.0175 days <br />0.419 hours <br />0.0025 weeks <br />5.741745e-4 months <br /> 25 April 1975.

The folicwing rcport is submitted in accordance with paragraph 6.7.2.a.4 (page G 11) of technical specifications.

On April 25, 1975, the D Reactor Protection System (!!PS) channel tri;: ped due to loss of pcstei.

As a result the undervolta';e coils to the Control Rod Drives (CRD) Secondary DC trip breakers were de-encrgized. Both breakers tripped.

In restoring the CRD system breakers to normal, an atte pt t.sas made to close the tuo DC breakers before the undervoltage enils were re-energized. One breaker closed, the cther breaker tripped free (did not close).

Both breakers should have tripped free in the presence of a trip signal. CC-4 is the dasignatica cf the breaker that =alfuncticned Both breakers were trip tested using the logic test switches in the T.PS Cabinet.

Both treakers responded correctly to the RPS trip signal, tharsby indicating that the breakers would opc 1 in the event a trip signal was received frcm the RPS Bistable Cabinct.

An investigation was made into the reason for the CB-4 treaker's ability to be closed with the undervoltage coil de-energized.

It was determined that the trip linkage from the undervoltage coil was out of adjustment.

The screw adjustment tras made in accordance with the vendor tech manual and the CB-4 breaker was satisfactorily trip tested.

In addition, it was verified that CS-4 would not close with a trip signal present prior to re-turning CB-4 to service.
