and J.G. Herbein ( TMI-Station Superintendent) at 1500 hours0.0174 days <br />0.417 hours <br />0.00248 weeks <br />5.7075e-4 months <br /> 26 Decemtmr 1974. .

Tim follwimi report is submitted in acconlance with paragraph G.7.2.4.2 (page 6-11) of the technical specifications.

On 26 Decm.bar both doors of the Peactor Dullding Personnel Acenss Hatch were opaed sirasitaneously while the plant was at 99.35% pow 2r (2513,2 IWt).

The outer door of the Reactor Building Personnel Access Hatch was inadver-tently left open following personnel egress from the Ranctor Bui~ldirg after a routine intpectica, Tire outer door ws dt scovered ajor een th- Irmer door was opened on subscrivant personnel curess. Upon discovery that both doors werw open, both doors wre imediately cloud.

Subminent investigatien indicated that trough the outer door was open, ttm latching har associntM wt th the door closing ratchaniwt int.criock and alam was it7 the closed position.
