In accordance with Technical Specification 6.9.2. A.2, we are reporting a viola tion of Tech Socc 3.6.1.

3 At 1030 hours0.0119 days <br />0.286 hours <br />0.0017 weeks <br />3.91915e-4 months <br /> on 19 January 1978 during a Plant Operations Review Comittcc meeting it was recognized that on certain previous occasions contain-icnt integrity was violated.

The valve, BS-V47A and B, an outside contain"ent test vent valve wu opened to verify that no leak. age occurred past BS-V1 A and B following testing of BS P1 A and B.

Although there was no actual comanica-tion between inside and outside of containr'ent while these valves were opened the event represented a violation of containment integrity.

Technical Specification 1.7 defines, in part, that Containr4nt Integrity exists when the all non-autocatic containmant isolation valves and blind flanges are closed as required by the "Contalonent Integrity Check, List" attached to the 1

operating procedure "Contairmnt Integrity and Access Linits". 85-V47A and B are included in this procedure to be closed and therefore by definition contain-rent integrity was violated when these valves were opened. A ter,porary change to the procedures will be made to teminate the operation of BS-V-47A and B.

Further corres.ive action will be evaluated and discussed in the followup report.
