This teTegram is to confirm a conversation between L. Spessard (Region I-CRO),

and J.G. Herbein {TM1 Superintendent) at 1630 HRS. 5 Jm2e 1974.

The folicxing report is suhaitted in accordance with paragraph 6.7.2.a.5

{page 6-11) of ths Technical Specifications.

On 4 June 1974 preyfous revisions of valve checklists and startup checklists of 15 currently revised operating precedures wre found to have been used during the course of picnt startup. At 161S HR$. 5 June 1974 the Plant Operations Review Cc::afttee etaluated the occurrerm es en observed inadequacy in the irplicentation of adainistrative controls daring the cperation of the facility which could effect the safety of operatioTrs.

At the tim of the findirig the prinary plant was at 5320F,2155 psig and preparat!ws were fa progress for initial criticality, The specific differen;:es between the current revisfons and previcus revisions of the respective checklists were researched and no unsafe conditions were I found to exist as a result of using the outdated checklists.

All the rertsed valve Tineups were verified in the plant to assure they were resitfoned properly in accordance with the current procedures and a6:inistrative steps are befog taken to preclude similar adairdstrative errors.
