A meeting was held at the request of Nuclear Assurance Corporation (NAC) et t
Silver Spring, Maryland, on September 24, 1987, concerning the Model No. NAC-1 cask.
Discussion On September 2,1987 NAC met with NRC staff members to discuss two unrelated incidents with Model No. NAC-1 casks (see memo, Odegaarden to Files, dated 9/28/67).
In the first incident a cask cavity vent valve was accidentally left open due to operator error. Because of the nature of the fuel being trans-ported (dry) and the closure of the value box by a cover, no significant radio-l activity was released, s
NAC showed to the NRC staff a markup of the valve box on the Ldel No. NAC-1 cask. NAC is proposing to add a one piece rod and handle to the closure values on the cask such that if the valve box cover is secured, the valves must be in the closed position. The NRC staff found this to be an acceptable addition to the cask design to reduce operator error. The modification will require an amendment to the cask Certificate of Compliance.

In a second incident, the lid of an " empty" cask lid was welded closed by DOE i
because of primary lid gasket failure.
(The DOE facility lacked provisions to l
remove the lid and replace the gaskets.) NAC discussed steps they are planning
)
to take to restore the cask to service.
Because of the welding and subsequent weld removal the cask no longer meet the drawing dimensions required by the package certificate of compliance.
4 The areas an the cask sealing surface (Serial No.1-E) which were under[ cut during welding removal will be dye penetrant checked for cracks.
If the surface is free of cracks, the surface will be built-up by welding and then
]
remachined to the original surface dimensions and surface finish. The surface
]
will again be dye penetrant checked for cracks. The lid on the cask will be built-up in certain areas and a bevel will be machined into the Serial No.1-E 1
lid. The package drawing will be revised to show this modification as an option. This modification will also require an amendment to the Certificate 3
