.g ATTACHMENT K Description of Quality Assurance Program for Type B Quantity Shipments af Radioactive Material 1.
Organization a.
Dayton XRay Company pursuant to 10 CFR 71.12 and 10 CFR 71.51 (a) is responsible for a QA program inr Type B quantity r.hipments and transportation of radioactive material. Fabrication and design of Type B packages are not included.
b.
The organizational structure in Attachment H is applicable.

The Radiation Safety Of ficer. is responsible for administration of the
- program, training of per,annel, procurement of manu f acturer's certifications, maintaining packaging and shipping docon.mts, retratien of records and program audits.
2)
Radiographers are responsible for handling, storage, shipping and transporting radioactive packa Attachment E; and they are~ges in accordance with the procedures in responsible for inspecting, survey.ng and maintenance reports in accordance with the procedures in Attachment E.

Quahty Assurance Program a.
Radiographers are trained in their responsibilities above according to A t tachmen t F.

The QA program will be viable, requiring management approval, and the program encornpasses spec ific requirements stated in the packa;;e approv..! for i
use, and general radiation safety and shipping requirements.

Manufacturer's certifications will be requested anel maintained on all NRC Type B packages manufactured and procured af ter January 1,1979.

Document Control a.
Packaging,. shipping, inspection and handling instructions from the manufacurers will be maintained for each model Type B package.
b.
The RSO shall insure that the shipment and transportation of Type B packaacs is conducted in accordance with the above documents.
4.
Handling, Shipping, Transportation and Storage a.
Radiography personnel will handie, ship, transpo,rt and store racmetive packages in accordance with the procedures in Attachment E.
b.
Radiograph personnel will determine the completeness of packege inspections and pr sence of manufacturer's certificates before shipment.

in accordance with Attachment E procedures.

Quarterly inspection and maintenance of Type 3 pachc;es will be performed by the RSO as outlined in Attachment H.

Equipment and package, procedures and instructions are maintained by the Radiation Safety Of ficer.
7.
Aduits a.
Quarterly audits of the entire radiation safety program is performed in accordance with Attachment C.
b.
The quarterly aduits include the necessary checks on the QA program as outlined in Attachment H.
