Reportable Significant Deficiency

Dear Mr. O'Reilly:
On October 17, 1979 Mr. Jack Skolds of NRC Region II was notified of a potqptial significant deficiency.
Under the South Carolina Electric and Gas Company program this defect which was determined, on March 4 1980, to be a 2
substantial safety hazard as defined by 10CFR Part 21 is being reported as'a
'cignificant deficiency under 10CFR'50 55(e)
In our preliminary evaluation, consideration had been given to the fact that redundant motor driven emer-gency feedwater pumps would prohibit this failure from being a substantial safety hazard; however, following discussions with Region II on March 3, 1980 regarding the use of redundancy in evaluation of substantial safety hazards, SCE6G reevaluated the deficiency.
Mr. Jack Skolds of NRC Region II was no-tified on March 4, 1980 of this change in evaluation.
Details of the problem are discussed in the attachment to this letter.

Identification of Basic Component Emergency Feedwater Pump Drive Turbine, TPP-8-EF.

Nature of Defect, Safety Hazard Created, Evaluation The component involved is the emergency feedwater pump turbine which l
was manufactured by Terry Corporation and supplied through Bingham l
Willamette.
The turbine is a type GS-2, horizontally split.casingi l
tangential flow turbine fitted with a Woodward type PC-PL governor and equipped with a horizontally mounted steam control valve, The defect reported involved the potential for the governor valve to close when it-~
should remain open while the turbine is on stand-by and being subjected to excessive vibration.
If the problem remained uncorrected it could be possible that during an
. accident the turbine driven emergency. feedwater pump would not be avail-e -.
able.
If there was a concurrent single active failure of one of the 50%
capacity motor driven pumps, this failure could possible affect the ability to conduct a safe shutdown.

Date Information of Defect was Obtained Terry Corporation letter to NRC dated Noverber 6,1979 identifying de-feet as a 10CFR 21 item.
6.
Number and Location of Defects There is only one coergency feedwater pump drive turbine, TPP-8-EF, in the V. C. Summer Nuclear Station.
7.
Corrective Action The corrective action taken was to modify the governor linkage to
^i accommodate a spring which preloads the valve linkage so that the governor valve remains in the open position during stand-by.
The modification was completed during February,1980 utilizing a re-trofit kit and instructions supplied by Terry Corporation.
