V Appendix A NOTICE OF VIOALTION Illinois Power Company Docket No. 50-461 Based on the results of the NRC inspection conducted on January 29, February 1 and 4-5, 1980, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below. These items are infractions.

10 CFR 50, Appendix B, Criterion VI; states in part that " Measures shall be established to control the issuance of documents...
in-cluding changes thereto, which prescribe all activities affecting quality."
CPS Quality Assurance Manual, Chapter 6, Section B (General), Para-graph 5, states that " Measures shall be established to assure current revisions of documents are distributed to the point, of use and are used."
Contrary to the above, licensee failed to provide measures for obsolete or superceded documents against inadvertent use, in that-a superceded calibration instruction No. 009, Revision dated March 8, 1979, had not been replaced in BA QA Manual Copy No. 4.

10 CFR 50, Appendix B, Criterion XVI states in part that " Measures shall be established to assure that conditions adverse to quality, such as...., nonconformances are promptly identified and corrected.
In the case of significant conditions adverse to quality, the meas-ures shall assure that the cause be determined and corrective action taken to preclude repetition".
Clinton PSAR, Chapter 17, Section 17.1.16.1, states, in part, that "A corrective action system shall require that conditions adverse to quality be promptly ident-ified, the cause' determined, and action taken to preclude repetition."

Contrary to the above, the licensee did not take prompt corrective action to preclude repetition of nonconformances, in that an exces-sive number of NCR's were allowed to occur prior to taking corrective action such as CAR's No. 048, 042, and 040.
