Dravo Corporation Docket No. 99900017/79-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on November 27-29, 1979, it appears that certain of your activities were not conducted in accordance with ERC requirements as indicated below:
Criterion V of Appendix B to 10 CFR 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Deviations from these requirements are as follows:

Paragraph NB-4232(a) in the ASt1E Section III Code states in part,
" Alignment of sections shall be such that maximum offset of the finished weld will not be greater than the applicable amount listed in Table NB-4232-1.
Table NB-4232-1 lists the maximum 0.D. offset for section thickness 3/4 inch to 1 1/2 inch as 3/16 inch.
Contrary to the above, the inspector observed an offset of 1/2 inch on weld joint C, a 10 inch schedule 140 pipe, sketch number 2936-1212, serial number 19018, and the assembly was completed and accepted, except for application of the ASME Code stamp.

Section 9 of the QA Manual, paragraph 9.10.6, states in part, "The immediate area around the temporary attachment shall be marked so that after removal the area can be identified until the appropriate NDE has been performed."
Contrary to the above, the immediate area around the temporary attachment was not marked on four (4) pipe sub-assemblies, sketch numbers E3037-691, E3037-196, E3037-669, and E3037-181 so that after removal the area could be identified. The temporary attachments had been removed and welds ground flush with the pipe and the NDE had not been performed as of the date of this inspection.
