NOTICE OF VIOLATION Tennessee Valley Authority License No. CPPR-163 Phipps Bend Based on the NRC inspection December 11-14, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspon-dence to you dated December 31, 1974.
As required by Criterion V of Appendix B to 10 CFR 50, implemented by the PSAR Section 17.1A.5, " Activities affecting quality shall be prescribed by documented instructions, procedures or drawings... and shall be accomplished in accordance with these instructions, procedures, or drawings." Procedure QCI-C-401 specifies that each cadweld splice shall be visually inspected for allowable voids in filler metal and if unacceptable, identified and tagged in accordance with procedure CEP-15.01.
Contrary to the above, on December 13, 1979, cadweld splice AB-18-VB-C029 which had passed QC acceptance was found to have an accumulative void area of 3.66 square inches which exceeded the allowable 3.0 square inches as specified by Procedure QCI-C-401.
This is a deficiency.
