7 50-341/78-16. This repo-t covers a two + art inspection of Enrico Fermi Unit 2 Site Construction activities performed by Mr. E.W.K. Lee on September 27 and 28, and October 11, 12, and 13, 1978.

Only the cited infraction mentioned in your report is discussed in this reply as required by Section 2.201 of the NRC's " Rules of Practice",

Part 2, Title 10, Code of Federal Regulations. The unresolved items have been acted on. We will be prepared to report in detail on our progress and corrective action on these matters to your inspectors on their next visit.

App'endix A and Par,agraph 1 Infraction: Contrary to 10CFR50,' Appendix B, Criteria XVI, the

to the removal of defects on Weld No. E21-3147-0W2 on Drawing No. 6M721-3147-1. This is a repe,titive item that was identified previously in IE Reports No. 50-341/

7B-08 and No. 50-341/78-11.

Corrective Action Taken and Results Achieved i

Since this violation was'similar_in_ nature to previously identified prob-4 lems, Wismer and Becker's Quality Manager' issued '5 Halt Wbrk directive, g stopping all.Wismer and Becker welding activities. In addition, a hold a tag was placed on the veld joint.

In order to resume safety-related welding, all welders were required to attend a training session. Upon completion of the training, welding was resumed with. one Q.C. inspector monitoring- each welding operation. This was continued until the-Wismer and Becker Project Quality-Manager-waa satisfied that the craf t were performing in accordance with the instruc-tions given in the training classes.

As for the weld, Wismer and Becker's Welding Engineer evaluated the condi-tion of the root opening and generated a " Weld Repair Record" (W & B Form I-6) to direct repairs to the groove. Subsequently, the weld was repaired.

Corrective Action Taken to Avoid Further Non-compliance At the time of the inspection the root pass had been made and the Wismer and Becker Q.C. inspector had performed a visual inspection, accepting the root veld. Dur,ing subsequent welding operations involving fill passes, the welder had ground through the root while removing an area of porosity, thus resulting in a configuration of the root opening which deviated from that specified in the welding procedure. However, at this time, the welder erroneously thought he was ready to continue welding.

Therefore, it was determined the primary cause o'f this problem was the welder's inability to identify the condition as being in non-compliance with the procedural requirements and to notify appropriate personnel.

Although the Wismer and Becker Q.C. inspectors performed surveillance of the fill pass welding operations, they had not detected this particu-lar deviation. , ,

' To prevent this type of' problem from recurring, a three-part training program was developed by Wismer and Becker, which consisted of classroom instruction in the following subjects: .

1. Weld procedures / Specifications and how to use them
2. Weld repair
3. Necessary actions to be taken by all parties when Weld Parameters are not met The lesson plans were reviewed and approved by Edison and Daniel Q.A.

The training was directed at the welders, but all supervisory and Q.C.

personnel attended the sessions. At the conclusion of each part of the training session a questionnaire was completed by the attendees to de-termine the effectiveness of the. training.
