Appendix A NOTICE OF VIOLATION Hammond Sanitary District License No. 13-17232-01 Based on the inspection conducted on November 5, 1980, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.
Items 1 through 3 are infractions, and item 4 is a deficiency.
1.
License Condition No. 12 requires that licensed material shall be used by or under the supervision of a named individual.
Contrary to the above requirement, it was determined through state-ments of licensee representatives that this requirement is not being met.
Specifically, since November 26, 1978 the gas chromatography unit has been used by an individual who is not authorized.

License Condition No. 16 requires that licensed material be possessed and used in accordance with statements, representations and procedures contained in application dated Sep* ember 30, 1976.
Item No. 5 of the above referenceu application names Michael T. Unger as the radiation protection officer.
Contrary to the above referenced application, it was determined through statements of licensee representatives that the individual designated as your radiation protection officer terminated employment on November 26, 1978.

License Condition No. 15A requires that you test your sealed sources containing byproduct material for leakage and/or contamination at intervals not to exceed six months.
Contrary to this requirement, as of the date of the inspection, you had not tested your sealed source containing 10 millicuries of nickel-63 for contamination or leakage since the issuance of this license, December 13, 1976.

10 CFR 19.11(a) and (b) require that current copies of Part 19, Part 20, license conditions, documents incorporated into the license, license amendments and operating procedures be posted, or that a notice describing these documents and where they may be examined, be posted.
10 CFR 19.11(c) requires that a Form NRC 3, " Notice to Employees" be posted.
Contrary to this requirement, on the day of the inspection, November 5, 1980, neither the documents nor the notices were posted.
