License No. 25-17317-01 NOTICE OF VIOLATION Based on the results of the NRC inspection conducted on September 17, 1980, certain activities appear to be in noncompliance with NRC regulations and the conditions of your NRC License No. 25-17317-01 as indicated belcw:
1.
License Condition 12 requires that licensed material be used by, or under the supervision and in the physical presence of, specified individuals.
Contrary to this requirement, individuals other than those specified were supervising use r# material at the time of the inspection.
This is an infraction.
2.
License Condition 17 requires in part that licensed material shall be used in accordance with statements, representations, and procedures contained in the letter dated March 3,1977, Item 2 of which st.es that the portable survey meter will be returned to the manufacturer for calibration every six months.
Contrary to this requirement, such a calibration of the Ludlum Model 3 survey meter had not been performed since it was purchased on or about January 1,1978.
This is an infraction.
3.
License Condition 13.B requires in part that records of sealed source leak test results shall be maintained for inspection by the Commission.
Contrary to this requirement, records were not maintained for such tests performed between May 12, 1977, and April 13,1978, for a 3-curie americium-241 sealed source, serial number T356.
This is a deficiency.

10 CFR 20.205(c)(1) requires that each licensee, upon receipt of a package containing quantities of radioactive material in excess of the Type A quatities specified in 10 CFR 20.205(b)(2), other than those transported by exclusive use vehicle, shall monitor the radiation levels external to the package.
Contrary to this requirement, such monitoring was not perforred uoon receipt of 3-curie americium-241 sources on June 29, 1977, and June 24, 1980.
This is an infraction.

10 CFR 19.11(a)(b) requires that eacn licensee shall post current copies of specified regulations and the license or a notice specifying where such documents may be examined.
Contrary to this requirement, neither the documents nor a notice were posted at the time of the inspection.
This is a deficiency.
6.
10 CFR 19.11(c) requires that Form NRC-3, " Notice to Employees," snall be posted by each licensee wherever individuals work in or frequent any portion of a restricted area.
Contrary to this requirement, such a form was not posted at the time of the inspection.
This is a deficiency.
