License No. 49-18179-01 NOTICE OF VIOLATION Based on the results of the NRC inspection conducted on April 8, 1980, certain activities appear to be in noncompliance with NRC regulations and the conditions of your NRC License No. 49-18179-01 as indicated below:
1.
License Condition 13 requires sealed sources possessed under the license be leak tested at intervals not exceeding six months.
Contrary to the above, a leak test was not performed on a cesium-137 sealed source (SN 5476) during the period from August 23, 1978 through December 14, 1979.
This is an infraction.
2.
10 CFR 20.407(b) requires a statistical report of personnel monitoring information recorded for individuals for whom personnel monitoring was required or provided be submitted to the Director of Management and Program Analysis, U. S. Nuclear Regulatory Commission, Washington, D. C. 20555 within the first quarter of calendar years 1979 and 1980.
Contrary to 10 CFR 20.407(b), statistical reports of personnel monitoring were not submitted to the Director of Management and Program Analysis, U. S. Nuclear Regulatory Commission, Washington, D. C. 20555 during the period January 1, 1979 to April 8, 1980.
This is a deficiency.
