Thank you for the 30-day report dated August 22, 1980, and comments from former employees which you submitted in regard to the lost gauge contain-ing an 88 millicurie cesium-137 se led source which was used in your plant at 3341 Jennings Road in Cleveland, Ohio.
It appears that you made a reasonable effort to contact former employees and therefore, we plan no further action regarding this matter.
- However, should you receive any new information cone raing the lost gauge, please notify this office immediately.
Our office also received your completed Form NPC-314, " Certificate of Disposition of Materials" and associated transfer papers regarding the other three gauges which you returned to Kay-Ray, Inc. in Arlington Heights, Illinois at the end of August, 1980.
Your copy of License No. 34-16555-01 is being returned to you since we have our own copy of this license.

Description of licensed material:
Kay Ray Model 7062 Source Housing for Continuous Level Measurement.
Cesiu:.137; about the size of an eraser on a new pencil; encapsulated in aluminum tubing which is embedded in lead shield, (88 millicuries).
2.
Circumstances of loss:
Unit was removed from service due to inability to get thc shutter open.
Plant manager failed to return the old unit to the manufacturer expeditiously and placed it in the electrical store room, on or about Dec.1,1979.
3.
Statement of probable disposition:
Our opinion is it was erroneously discarded with cIner scrap / trash and is buried in the TESTA Landfill, Richmond Rd; Solon, Ohio.
4.
Possible radiation exposure hazard:
Not probable since the shutter is closed and extremly difficult to open without special tools because the opening handle is missing and most of its shaft is gone.
5.
Actions taken toward recovery:
Extensive searches of plant and premises,and also scrap yard in the preser te of Stanley Lasuk of N.R.C.,with his radiation detector (THYAR 'I Model No.489) were futile.
6.
Procedures taken toward recurrence of loss:
None taken since plant which uses these devices has been shut down and the Sources will be returned to the manufacturer.

The operators cleaned up and I never saw it again.
J0E MCINTYRE Remembers seeing it in front office. Nothing from then on.
