Appendix A NOTICE OF VIOLATION Pittsburgh-DesMoines Steel License No. 14-01837-04 Company Based on the inspection conducted on August 25 and 26, 1980, it appears that certain of your activities were in noncompliance with NRC require-ments, as noted below.

Items 1 and 2 are infractions, and items 3 and 4 are deficiencies.

1.

License Condition No. 19 states, in part, that the licensee shall possess and use licensed material in accordance with statements, representations, and procedures contained in the letter dated November 3, 1977.

Item II of the periodic training section of this letter requires, in part, that refresher training be con-ducted annually or more often as needed.

Contrary to this requirement the licensee has failed to perform such training.

License Condition No. 19 states, in part, that the licensee shall possess and use licensed material in accordance with statements, representations, and procedures contained in the letter dated November 3, 1977. An administrative section of this letter entitled

" Internal Inspection Program", requires, in part, that the assistant division radiation safety officer perform an unannounced internal inspection at least quarterly.

Contrary to this requirement, as of the date of this inspection, the licensee has failed to conduct such quarterly inspections since November 1,1978.

10 CFR 34.27 requires, in part, that the licensee maintain current logs, showing for each scaled source the following information:

A description of che radiography exposure device or storage a.

container in which the sealed source is located b.

The identity of the radiographer to whom assigned c.

The plant or site where used and dates of use 8009250 s

Pittsburgh-DesMoines Steel Company Contrary to the above requirements, the licensee has failed to include the plant, site or physical location of such radio-graphic operations on the utilization log.

4.

10 CFR 34.26 requires, in part, that the licensee shall conduct a quarterly physical inventory to account for all sealed sources and maintain records of such inventories which include the quantities and kind of byproduct material, location of scaled sources, and the date of such inventories.

Contrary to these requirements the licensee has failed to indicate the location of such sources on the quarterly inventories.
