In response to your letter of January 3,1980, please be advised that the following steps have been taken to assure positive control over source control document completion:
1.
Internal note, dated November 16, 1979, that notes deletion of Form NBS-366, " Record of Inspection for Radioactive Material".
2.
Memoranda to NBS Division Chiefs and to Health Physics staff, both dated November 16, 1979, with attachments, that restate the required procedures.
3.
Memorandum to Health Physics staff, dated November 26, 1979, with attachments, and Interim Procedures for Ionizing Radiation Source Control, dated NovemUer 20, 1979, that show modified forms and specify appropriate actions for source control.
4.
Memorandum in final form to NBS Division Chiefs, dated December 3, 1979, with attachment, that specifies appropriate actions for source control.
Copies of these memoranda, attachments, and procedures are enclosed.
The results to date are that more Health Physics-to-user interaction is re-quired to satisfactorily complete the source control documents.
In addition to the actions which have been taken, a periodic audit of control documents, at intervals not to exceed one month, will be made by the Supervisory Health Physicist to insure satisfactory performance of documentation procedures. Also, the listing of authorized users and sources for which they are authorized is continuing.
The auditing process will continue until satisfactory operation is routine, but for no less 8002200 2C b
2 than twelve months. The listing of authorized users and sources should be completed by July 1, 1980, although a continuing update program will be necessary.

Chief, Occupati a Health & Safety Division Subj ect Control of Radioactive Materials This memorandum is to remind each user of radioactive materials or radiation-emitting machines that authorization to utilize these materials or equipment or to use any materials or e.quipment with incidental involvement with radio-activity must be authorized through the Fom NBS-364, Proposal to Acquire Radiation Source, or Fom NBS-365, Proposed Change in Utilization of Radiation Source.
An authorization from Health Physics, via the fom must precede any acquisition or change.
All numbered items on the appropriate fom must be completed in detail to be approved.
In partisular, items 5 and. 6 must list detailed source. descrip-tions and detailed use parameters, including facilities to be used, safety precautions to be employed, and operating procedures.
So that adequate infomation is available to Health Physics for evaluotion of proposals, please prepare a list of qualified users in your organizatica, with summary sketches of academic training and work experience, and a list of facil-ities and equipment which will be evaluated.
These lists should be submitted to Health Physics by December 17, 1979.

In checking sources of radiation to co=plete authorizations for use and utilizations, complete all unnumbered items on Forms NBS-364 & 365. Lists of qualified users cnd approved facilities and equipment will be on file in the Health Physics office.

Receiving Room ticket signed when a package is accepted for. Room B13.1 or the date equipment is received by the user.
The last step in the approval is obtaining the signature of the approved custodian and the date transferred from Health Physics to him/her.
Initials of the surveyor of the package smear check and external survey must be entered.
RS numbers will be kept sequentially in a journal under Ms. Coker's control.

IONIZING RADIATION SOURCE CONTROL INTERIM PROCEDURES - 11/20/79 1.
Purpose:
To establish and maintain control of radioactive materials and ionizing radiation producing equipment for the radiation safety program at NBS.
2.
Scope:
All radioactive materials at NBS except materials under the NBSR license and all equipment at NBS that produces ionizing radiations except producers of x-rays with maximum enerC es less i
than 25 KcV, e.g., cathode-ray tubes, are to be authorized by Health Physics.
3.
Source Acquisition:
-(a)
Forr NBS-364, " Proposal to Acquire Radiation Source", or otbc dccument specifically approved by Health Physics shall he used to document source acquisitions.
(b)
Sources lacking prior authorizations will be bandled by one of the following means:
(1) The source will be impounded or otherwise rendered inoperative by Health Physics until authorized as in (c) below, or (ii) interim authorization may be made by the Health Physics i
staff member in attendance until authorization is made as in (c) below.
This authorization requires tLaely control of the source use, including storage and use access, procedures, and associated facilities and equip-ment, by the authorizing staff member, and the Health Physics staff member will document the ir.terim authoriza-tion.
(c)
Authorizations will be by the Chief of Health Physics or specifically designated representatives.
