States Ter=inal Block, Cracked Disconnect Links INTERIM REPORT: 3 DATE: January 9, 1980 PROJECT: Consu=crs Power Co=pany Midland Plant Units 1 & 2 Bechtel Job 7220 INTRODUCTION

'Ihis report is sub=itted to provide the interi= status and current course of action required pursuant to MCAR 32 and Bechtel NCR 2367.

DESCRIPTION OF DISCRIPANCY Refer to Interi= Report 1 (revised) dated August 10, 1979.

PROBABLE CAUSE Refer to Interi= Report 1 (revised) dated August 10, 1979, for a description of potential causes of the cracked dis-connect links. The actual cause for the failures has not -

been definitely deter =ined; however, the =anufacturing process and the type of =aterial used in the process is still considered as the =ost probable pri= arf cause.

CORRECTIVE ACTION Since the last interi: report the following action ite=s were acted upon:

1) "Bechtel developed an inspection plan for deter =ining the extent of cracked links. in the =ajor equip =ent using states ter=inal blocks. The inspection began on Nove=ber 26, 1979, and is scheduled for co=pletion by February 1, 1980.

2) bB5tb kP.C5 and Multi-A=p (States) have investigated, via additional testing, the findings of each others' prelie.inary cetallurgical analysis. As of this date, no conclusive results are available f tom these tests, however, evaluations are continuing. In addition, further evaluations are being conducted to determine the feasibility of icplecenting additional tests to identify the cause of the cracks and establish cor-rective action.

SAFETY IMPLICATIONS Refer to Interic Report 1 (revised).

FOPICAST DATE FOR CORRECTIVE ACTION A schedule for completion of the corrective action was to be provided in this report. However, due to the incomplete status of the following, a scheduled completion date cannot

1) Determination of the exact cause of the cr acks.

2) Completion of the inspection plan and evaluation of the results.

3) Establishing frasibility for implecenting any additional tests. .

A schedule for co=pletion of the corrective action vill be provided in the next report. ,,
