APPENDIX A NOTICE OF VIOLATION Defense Nuclear Agency Docket No. 50-170 Based on the results of an NRC inspection conducted on January 10-12, '979, it appears that certain of your activities were not conducted in full compliance with conditions of your license.
Items A and B below are infractions.
A.
Technical Specifications in Section I.A.1 require the reactor building ventilation to exhaust to a stack having a minimum elevation of 18' above the roof level of the highest building in the AFRRI complex.
Contrary to the above, on January 10 and 11,1979, a leak through a stack drain line discharged part of the exhaust at ground level outside the building.
B.
The Technical Specifications require adherence to written procedures and HPP 3-5 requires a minimum of 75 fpm face velocity at the Hot Lab Hood.
Contrary to the above, on January 10, 1979, with radioactive mate-rials present, the face flow velocity at the Hot Lab Hood was zero to 25 fpm.
