N Appendix A NOTICE OF VIOLATION Commonwealth Edison Docket No. 50-454 Company Docket No. 50-455 Based on the inspection conducted on January 23-25, 1979, it appears that certain of your activities were in noncompliance with NRC require-ments, as noted below.
This item is an infraction, part a is applicable to Unit 1 and part b is applicable to Units 1 and 2.
10 CFR 50, Appendix B, Criterion V states, in part, that, " Activities affecting quality shall be prescribed by documented instructions, procedures.
. and shall be accomplished in accordance with these instructions, procedures or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfac-torily accomplished."
Commonwealth Edison Company Topical Report No. CE-1-A, Revision 5, Section 5, states, in part, that "The quality assurance actions carried out for design, construction, testing, and operation activities will be described in documented instructions, procedures, drawings, specifi-cations or checklists. These documents will assist personnel in assuring that important activities have been performed.
These documents will also reference applicable acceptance criteria which must be satisfied to assure that quality related activity had been properly carried out."
Hunter Corporation Site Implementation Procedure No. 5.501, Revision 6, Paragraph 4.15 states, that "When the Welder has completed the welds that the material was drawn for, or when he has used up the rod on the requisi-ti,on, or at the end of the shift, he will return to the Rod Issue Attendant his butts, any excess weld material, and the yellow copy of the weld material requisition."
Contrary to the above, on January 24, 1979, the inspector established that:

A consumable insert for Weld No. 878 on Drawing No. CV-10-11 was issued to a welder on January 23, 1979, The insert was in the possession of the welder on January 24, 1979, and has not been consumed into the weld. Furthermore, evidence to indicate that the insert was returned to the Rod Issue Attendant at the end of the January 23, 1979, shift by the welder was unavailable, b.
Acceptance criteria have not been established in procedures or instructions for the gap opening of split backing rings used in ASME,B&PV Code,Section III, Class 3 piping.
