. License No. 43-12087-04 NOTICE OF VIOLATION Based on the results of the NRC inspection conducted on October 28, 1980, certain activities appear to be in noncompliance with the conditions of your NRC License No. 43-12087-04 as indicated below:

License Condition 13. A(1) requires that sealed sources shall be tested for leakage and/or contamination at intervals not to exceed six months.

Contrary to this. requirement,-

(a) A..Troxler gauge Model.10.5A,. serial number 23623E, containing a 100 millicurie americium-241 sealed source, was not tested between January 1,1978, and January 10, 1979.

(b) A Campbell Pacific sealed source, serial number 2566, containing 50 millicuries of americium-241, was not tested between October 18, 1979, and May 27, 1980.

(c) A Hewlett-Packard sealed source, serial number 6006003, containing 15 millicuries of nickel-63,- was not tested between October 1,1979, and May 5, 1980.

This is an infraction.
