APPENDIX A NOTICE OF VIOLATION

License No. 45-17195-01 Based on the NRC inspection August 14, 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in esrrespondence to you dated December 31, 1974.
A.
As required in part by License Condition 13(a)(2), each sealed source containing licensed material shall be tested for leakage and/or con-tamination at intervals not to exceed six months.
Contrary to the above, four sealed sources of 150 millicuries each of Americium-241 possessed by you were not leak tested during the period July 13, 1977 thru April 11, 1978, thereby exceeding the six month frequency required for such tests. Records showed that the sources were in use during this period.
This is an inf raction.
B.
As required in part, by 10CFR20.401(b), records shall be maintained showing the results of surveys required by 20.201(b). 20.201(b) requires that each licensee shall make or cause to be made such surveys as may be necessary for him to comply with the regulations in this part.
(1) Contrary to the above, records of surveys required to comply with 10 CFR 20 were not adequate in that the identity of the licensed material and the individual performing the survey were not shown.
(2) Contrary to the above, records of surveys required by 10 CFR 20.205(c) on incoming shipments of licensed material were not maintained.
This is a deficiency.
C.
As required in part by 10CFR30.51(a) each person who receives byproduct mate-rial pursuant to a license shall keep records showing the receipt of such byproduct material.
Contrary to the above, records kept by you for sealed sources of Americum-241, serial Numbers 7-389 and T-390, did not show the date of receipt.
This is a deficiency.
