The purpose of this letter is to supplement the information in the referenced letters by complying more specifically with the format outlined in 10CFR, part 21.

1. Name and address of the individual or individuals informing the Commission -

1.1 Our letters of September 11 and September 12 were from Herbert H. Henkel, Director Technical Services, Hilti, Inc., P. O. Box 45400, Tulsa, Okalhoma 74145.

1.2 This letter is from Michael Friedman, Vice President Marketing, Hilti, Inc., P. O. Box 45400, Tulsa, Oklahoma 74145;. Mr. Henkel is in Europe, participating in the investigation relative to to this matter.

2. Identification of the facility - At this time, we know that the products in question have been used at the Grand Gulf Nuclear Station, Port Gibson, Mississippi.

We are investigating to determine if these products were used in any other nuclear power plants.

3. Identification of the firm constructing the facility -

Bechtel Power Corporation, Architect Engineers represent-ing Mississippi Power and Light. Hilti, Inc. supplier of Kwik-Bolt Anchors.

4. Nature of the defect - The nature of the defect was covered in our letters of September 11 and September 12, 1980.

5. The date on which the information of such defect or failure to comply was obtained - September 11, 1980.

6. The number and location of such components in use -

Until the cause for the reduced product performance has been identified, we will not be able to determine the number and location of all such components in use. This investigation is being aggressively pursued.

7. The corrective action being taken - Additional in-house testing was conducted following our letters of September 11 and September 12, 1980. Thus far, we have been unable to determine the cause. Further investigation is being conducted at Hilti's Development facility at this time.

8. Information given to purchasers or licensees - Copies of our letters of September 11 and September 12, 1980 have been forwarded to Mr. Adrian Zaccaria, Project Engineer, Bechtel Power Corporation for Grand Gulf Nuclear Station, 15740 Shady Grove Road, Gaithersburg, Maryland 20760. A copy of this letter will also be provided to Mr. Zaccaria.

Every action is being taken to identify the cause for the reduced product performance outlined in the referenced letters.

As soon as the findings of this investigation are complete, they will be reported.
