License No. 44-165:9-01 NOTICE OF VIOLATION Based on the results of an NRC inspection conducted on May 13, June 6, and June 12, 1980, it appears that certain of your activities were not conducted in full compliance with NRC regulations and the conditions of your license as indicated below:
A.
Condition 17 of your license requires that licensed materials be possessed and used in accordance with statements, representations and procedures contained in your application dated June 27, 1976, and in several letters, one of which is dated September 29, 1976.
The application dated June 27, 1976, includes your " Operating and Emergency Procedures Manual."

Section 5.1.4 of your " Operating and Emergency Procedures Manual" requires that pocket dosimeters be recharged at the beginning of each work shift.
Contrary to this requirement, on June 6, 1980, a radiographer at your Rahway, New Jersey facility performed radiography without recharging his dosimeter at the beginning of the work shift.
This is an infraction.

Item 6.1(a) of your letter dated September 29, 1976, requires that each radiographer be inspected by a member of your Radiation Safety Program at three month intervals.
Contrary to tnis requirement, as of the date of the inspection, June 6, 1980, three radiographers at your Rahway, New Jersey facility were not inspected between November 6, 1979, and May 27, 1980, an interval of more than three months.
This is an infraction. We note that you have corrected this item in that an inspection of these radiographers was conducted on May 27 and 28, 1980.

Section 4.1 of your " Operating and Emergency Procedures" requires that a " radiation report" be completed by the radiographer each day to serve as a record of physical radiation surveys, utilization log, dosimeter readings and daily inspection of the radiographic equipment.
Contrary to this requirement, on the date of the inspection, June 6, 1980, a radiographer performed radiography without recording the above information on a " radiation report" fo rm.
This is a deficiency.

10CFR 34.33(c) requires that pocket dosimeters be checked at periods not to exceed one year for correct response to radiation.
Contrary to this requirement, as of the date of the inspection, June 12, 1980, pocket dosimeters used by radiographers at your Rahway, New Jersey facility had not been checked for correct response to radiation.
This is an infraction.
