Appendix A NOTICE OF VIOLATION Twin Ports Testing, Inc.
License No. 22-15932-01 Based on the inspection conducted on August 18 and 19, 1980, it appears that certain of your activities were in noncompliance with NRC require-ments, as noted below. These items are infractions.
1.
10 CFR 34.24 states, in part, that each radiation survey instrument shall be calibrated at intervals not to exceed three months.
Contrary to this requirement, on the date of this inspection, it was learned that survey instrument S/N 4632 was calibrated on March 13, 1980 and used routinely by the licensee from June 17, 1980 until the date of this inspection, a period af ter which the calibration had expired.

License Condition 16 states, in part, that the licensee shall possess and use licensed material in accordance with statements, representations, and procedures contained in the letter dated March 20, 1980. Section F of the Operating and Emergency Instructions entitled " Transporting Sealed Sources to Field Locations, Packing of Exposure Devices and Storage Containers, Posting of Vehicle and Control of Sealed Sources During Transportation" requires, in part, that the maximum level of radiation on the exterior of transporting vehicles not exceed 2 mR/hr.
Contrary to this requirement, on the date of this inspection, the inspector measured 6 mR/hr on the exterior surface of the vehicle used by the licensee for transporting licensed materials.
3.
License Condition 16 states, in part, that the licensee shall possess and use licensed material in accordance with statements, representations, and procedures contained in the letter dated March 20, 1980.
Section K of the Operating and Emergency Instructions, titled " Procedure for Shipping and Receiving Radiographic Exposure Devices and Sources" requires, in part, that upon receipt a survey be performed on shipping containers.
Contrary to this requirement, the licensee has failed to conduct such surveys.
