Purpose.

To establish a Quality Assurance Program for assuring continuous compliance with Nuclear negulatory Commission (NRC) License, Rules and Regulations for Radiographic Operations in accordance with reference (a).

2.

Background.

In accordance with reference (b), GILMORE is authorized to utilize by-product sealed sources in radiography.

Reference (a) requires that each organization establish and maintain a Quality Assurance Program to assure compliance with the rules and regulations established by the NRC to prevent over exposure to individuals.

Reference (c) outlines the opera-ting and emergency procedures to be followed during Radiographic Operations and inspections and control.

3.

Responsibility.

The final responsibility for the Quality Assurance (QA) Program for 10 CFR Part 71 requirements rests with the Commanding Officer, USS HOWARD W. GILMORE (AS16).

The QA Program is implemented using the following organizational structure:

a.

The Radiographic Safety Officer is responsible for overall administration of the program training, certification, document control, and auditing.

The Radiographers are responsible for handling, stor-c.

ing, shipping, inspection, test and operating status and record keeping.

5.

Action.

Enclosure (1) outlines the Quality Assurance Program required by reference (a) and shall be implemented in USS HOWARD W.

GILMORE (AS16).

b.

Audits shall be conducted at frequencies delineated by reference (c), enclosure (16), or more often as determined by the Commanding Officer.

Re':ords shall be maintained and reports made to the Commanding Officer in accordance with reference (c), enclosure (16). Reports of deficiencies and of completed corrective action thereon shall be submitted within two (2) weeks of the date of inspection.

Quality Assurance Program.

a.

The Commanding Officer, USS HOWARD W.

GILMORE (AS16),

establishes and implements this QA Program.

Training, prior to engagement, for all QA functions is required in accordance with reference (c), enclosure (3). QA Program revisions will.

be made in accordance with reference (c), enclosure (1), with management approval.

The QA Program will ensure that all de-fined QC Procedures, engineering procedures, and specific pro-visions of the package design approval are satisfied.

The Radiographic Safety Officer shall assure that all radioactive material shipping packages are designed and manu-factured under a QA Program approved by Nuclear Regulatory Commission for all packages designed or fabricated after the effective date of the QA Program.

This requirement can be sat -

isfied by receiving a certification to this effect from th-manufacturer.

2.

Document Control, a.

All documents related to a specific shipping package will be controlled in accordance with reference (c), enclosure (9). All document changes will be performed according to written procedures approved by the Commanding Officer.

b.

The Radiographic Safety Officer shall ensure that all QA functions are conducted in accordance with the latest appli-cable changes to these documents.

3.

Handling, Storage and Shipping, a.

Written safety procedures concerning the handling, storage and shipping of packages for certain special forms of radioactive material will be followed.

Shipments will not be made unless all tests, certifications, acceptances, and final inspections have been completed.

Work instructions will be provided for handling, storage, and shipping operations.

Radio-logical Control and Radiographic personnel shall perform the critical handling, storage and shipping operations.

Inspection, Test and Operating Status.

a.

Inspection, test and operating status of packages for certain special forms of radioactive material will be indicated and controlled in accordance with reference (c), enclosure (10).

Status will be indicated by tag, label, marking or log entry.

Status of non-conforming parts or packages will be positively maintained in accordance with reference (c), enclosure (11).

b.

Radiological controls personnel shall perform the re ~

quired inspections and tests in accordance with reference (c).

The Radiological Controls Officer shall ensure that these functions are performed.

5.

Quality Assurance Records, s.

Records of package approvals (including references and drawings), procurement, inspections, tests, operating logs, audit results, personnel training and qualifications and records of ' shipments will be maintained.

Descriptions of equipment and written procedures will also be maintained.

b.

These records will be maintained in accordance with reference (c), enclosures (1) and (14).

The records will be identifiable and retrievable.

A list of these records, with their storage locations, will be maintained by the Radiological Controls Officer.

6.

Audits.

Established schedules of audits of the QA Program will a.

be performed using written checklists in accordance with reference (c), enclosure (16).

Results of audits will be main-tained and reported to the Commanding Officer.

Audit reports will be evaluated and deficient areas corrected.
