99900281/79-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on October 16-19, 1979, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:

Criterion V of Appendix B to 10 CFR 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Deviations from these requirements are as follows:

The Allis-Chalmers Valve Division QA Manual issued August 14, 1979 (Sec, tions N5.1, N5.2 and N6.4 ) requires QA/QC sign-offs for the following manufacturing documents / activities:

Routing sheets to assure that all required prior operations have been a.

perfo rmed. The inspectors stamp (or signature) and date is used to indicate acceptance, b.

Material Traceability Summaries. QC verification is required to assure that the identification of individual items in a fabricated assembly are properly logged.

c.

Weld Records. QC inspectors are required to monitor welding operations and indicate compliance with specifications by signing and dating the weld record.

Contrary to the above, sign-offs for the above activities were being made by manufacturing rather than QA/QC personnel.
