SUBJECT:

SAFETY IMPLICATIONS OF CONTROL SYSTEM FAILURES AND PLANT DYNAMICS Appendix 19 of the report of the President's Commission on the Accident at TMI contains the following statement on page 35:

"This PORV failure is a clear indication of the need for better configuration control and interface coordination.

It provides a good opportunity for a failure mode and effects analysis FMEA)."

(emphasis added).

Furthermore the conclusion of Appendix 19 on page 3 is:

"The TMI-2 accident would probably not have progressed beyond a severe feedwater transient, had the PORV been recognized and treated as a safety-related component."

I am citing these two points for your convenience and in support of statements and recommendathas I have made on the subject matter pre-viously, including my memorandum to you of December 20, 1979.
