['M[O 3/lOEfbb/NbMd' Director, Nuclear Peactor Regulation Att: Mr Dennis L Zie= ann, Chief Operating Reactors Branch N6 2 US Nuclear Begulatory Co==ission Washington, DC 20555 DOCKET 50-255 - LICENSE EPR PALISADES PLANT - EESPONSE TO IE INSPECTION REPORT 79-17 Consumers Power Company response to the alleged discrepancies ncted in IE Inspection Report 79-17 dated November 21, 1979, is as follows :

Item 1 - In fraction Technical Specification 3.8.1.g recuires a check of primarf coolant syste=

boron concentration each shift when refueling operations are being perfor ed in the reactor.

Contrarf to the above, the licensee failed to check pri=ary coolant system boron concentratien during the "A" shift on October 2,1979, a shift during which refueling operations were being perfomed in the reactor.

Restense to Item 1 Discussion The apparent violation of Technical Specification 3.8.1.g is attributed to two factors:

(1) oversight by the individual assisned to draw the sa.ple, and (2) failure of shift operating personnel to stop refueling operations when it was deter =ined that no boron sample was taken on the shift in cuestion.

Corrective Action Taken and Pesults Achieved The refueling check sheets for the 1979 refueling outage have been examined, and it has been verified that the PCS boren concentration during both the shifts i==ediately preceeding and following "A" shift on October 2,1979, was in speci-fication. Accordingly, it is concluded that the PCS boron concentration during "A" shift on October 2,1979, was also within specifications.

2 Consumers Power Company December 22, 1979 Corrective Action to be Taken to Avoid Further Noncomeliance This occurrence has been reviewed with the Chemistry Technicians, and the import-ance of obtaining required samples has been stressed.

This event will also be re-viewed with shift operating personnel, and the importance of verification of all refueling requirements during refueling operations vill be stressed.

Date When Full Compliance Will be Achieved The reviews described above vill be co=pleted prior to the next refueling outage.

Item 2 - Infraction Technical Specification 6.8.1.b requires implementation of procedures covering refueling operations.

Procedure FHSO-2, "Re fueling Procedure" includes checklist "the CL A5.1L-1, "Re fueling Operations Shift Check Sheet," which states, in part:

minimum spent fuel pool / reactor cavity level is one foot below the surface skimmers during fuel handling."

Contrary to the above, the licensee handled fuel on the "A" shifts of October 11 and 12, 1979, when the water level was 1.5 and 2 feet, respectively, below the ski==ers.

Besponse to T, tem 2 Dis cuss ien The failure to properly implement Procedure FHS0-2 resulted from a ec=tination of factors :

(1) a small leak in a steam generator da= cc=bined with a small leak in the reactor cavity liner resulted in the pool level dropping more rapidly than would be expected under normal refueling conditions; (2) the shift operating personnel failed to halt refueling operations upon discovery of this out of speci-fication condition.

Corrective Action Taken and Results Achieved Fepairs have been made to both the leaking steam generator dam and the reactor cavity liner such that these leakage paths should be eliminated. Shift cperat-ing personnel vill be instructed regarding the significant of verifying that prereq-uisites for refueling are within specifications.

Corrective Action to be Taken to Avoid Further Nonecmoliance Shift operating personnel vill be trained in refueling procedures prior to Palisades next refueling outage.

Date When Full Compliance Will be Achieved The required training vill be completed prior to the next refueling outage.
