Operating License No. DPR-73 Docket No. 50-320 Inspection Report No. 78_-12 This letter is in response to the subject investigation report resulting from Mr. Narrev's investigation of March 6 and 15,1978, an:1 the finding therect.

Selev is the response to the infraction referenced in the subject investigation.

Infraction 10 CFR 50, Appendix 3, Criterion V, requires, in part, that " Activities affecting quality shall be...accc=plished in acecr:iance with instructions,

proce:iures and drawings".

Paragraph 17.15.h of the quality Assurance Prcgram docu=ented in the FSAR states, in part, "....all quality related activities on structures,

syste=s and cc=ponents within the QA Program are perfor=ed in accordance with specificatien, precedure and drawing requirements."

Section 1 of United Engineers & Constructors' QC Precedure QA-8A-2, Revision 2, for velding centrol states that this procedure applies to velding, exclusive of pipe velding perfer=ed en nuclear and nuclear related areas and en Class I seismic structural building me=bers in the reactor buildirs.

Paragraph A k.a of Section IV of this procedure states, in part, " prior to velding, verify that approve:1 velding precedures...are to be empicyed in the work".

3. E. Grier, Director GQL 0811 Corrective Action Taken Atte= pts to verify the adequacy of the installed anchor plate proved futile due to the configuratica of the plate. Therefore, EDR hT0h was issued by the engineer which velded two additional 1 inch plates to the existing tank support plate. These additional plates ver2 then anchored to the vall utili::ing six 3/h inch expansion bolts. The engineer reviewed the possible effect of the reinforcing bar being cut both for the original installation and for the six new expansion bolts and deter =ined that no adverse affect on the integrity of the structure was experienced.

Cor ective Sters Taken to Avoid Further Violations Two additional anchors in the area which had the sa=e potential for experiencing the sa=e installation difficulties were checked by UT examinatica and verified to be installed correctly. A previcus review of hanger anchorages for correct stud length had been perfor=ed en site in an atte=pt to verify their acceptability. Over 100 hangers vere randc=ly selected to receive a UT inspection of their anchor stud lengths.

Only one stud was found to be shorter than that required by the engineer, hevever, the constructor had ec=pensated for this by placing 5 anchor studs in the henger plate instead of the h studs required by the engineer.

The history of anchor stud inspections conducted on site reinforces the state =ent

=ade by the alleger that this was a single isolated instance. We therefore conclude that no further corrective action needs to be taken for this cecurrence.
