W discovery that the water in the vessel head is at the liot-leg temperature rather than the cold-leg temperature as assumed in their calculation of LOCA/ECCS perforr.ance.
l 2.
Measurements by W that rod bowing requires derating more than allowed for in current models.
1 + 2 resulted in derating some operating plants.
l 3.
Discovery by CE of mistakes in their LOCA/ECCS codes that l
resulted in more restrictive power density limits on several j
reactors.

Surry Steam Generator tube failure which led to discovery i
of new failure mode and order to-6 plants to shut down within a month and inspect stear generator.
5.
Discovery of several ( N10) cracks in a S injection line from stress-assisted corrosion and indication that the problem may be generic.
