49-1731f-01 fl0TICE OF VIOLATION Based on the results of the flRC inspection conducted on December 4-5, 1978, certain activities appear to be in noncompliance with f1RC regulations as indicated below:
1.
Contrary to CFR 20.205(d), procedures for safely opening packages in which licensed material is received, had not been established on December 4, 1978.
This is an infraction.
2.
Contrary to 10 CFR 30.51, records of receipt of licensed material were not maintained on December 4, 1978.
This is a deficiency.
3.
Contrary to 10 CFR 20.401(a) records of quarterly doses of radiation exposure to individuals could not be determined due to unprocessed persorinel monitoring devices for the period April 1,1978 to June 30, 1978.
This is a deficiency.
4.
Contrary to 10 CFR 19.11(a) or (b), neither the documents required by 10 CFR 19.11(a), nor the notice required by 10 CFR 19.11(b) were posted on December 4, 1978.
This is a deficiency.
5.
Contrary to 10 CFR 19.11(c), Form f1RC-3, "flotice to Employees,"
was not posted at the restricted area where licensed material was stored on December 4, 1978.
This is a deficiency.
