License No. 40-02331-19 NOTICE OF VIOLATION Based on the results of the NRC inspection conducted on October 2-3, 1980, certain activities appear to be in noncompliance with the conditions of your NRC License No. 40-02331-19 as indicated below:

18 requires that the licensea shall possess and use lic-
!al described in items 6, 7 and 8 of this license in accordance with statements, representations, and procedures con-tained in part in the letter and application dated February 26, 1979.
Part G of the Radiation Safety Program requires that the Radiation Safety Officer perform monthly surveys of the facilities where radio-isotopes are used at University of South Dakota.
Contrary to this requirement, the Radiation Safety Officer did not perform monthly inspections during the months of May, August, November, December 1979, January, February, April, May, June, July, August and September 1980.
This is an infraction.
