Docket No. 99900038/79-02 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on September 4-5, 1979, it appears that one of your activities was not conducted in accordance with NRC requirements.
Criterion V of Appendix B to 10 CFR 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Paragraph 4.2.3, sub paragraph (5), in Section 4 of the QA Manual states in part with respect to vendor surveys, "
Any deficiency found in the survey shall be resolved by the vendor prior to being included in the Approved Vendor List Contrary to the above, Nippon Steel Corporation (Hikari k'orks) was included on the Approved Vendor List without prior resolution of the deficiencies identified during a survey performed on May 16, 1978.
It should be noted, however, that procurement from this vendor was not made until approximately 3.5 months after the deficiencies had been resolved.
