1.
Purpose. To establish a Quality Assurance program for industrial Radiography as required in title 10, Code of Federal Regulations, part 71.
I 2.
Organization.
a.
The final responsibility for the Quality Assurance (QA) program for part 71 requirements rests with SIl'A florfolk. Design and fabrication shall not be conducted under this QA Program.
The QA program is implemented l
using the following organization.

The Radiographic Safety Officer is responsible for overall 1
administration of the program, training and certification, document 4
control, and auditing.

The Senior Radiographer and Radiographers are responsible for handling, storing, shipping, inspection, test and operating status and record keeping, in accordance with references (a) and (b) and this instruction.

Quality Assurance Program. The establishment and implementation of this QA program is the responsibility of the Radiographic Safety Officer. Train-ing,' prior to engagement, for all QA functions is required.in accordance with written procedures established in reference (a) and QA program revisions will be made in accordance with written procedures and with the QA Officer's approval. The QA Program will ensure that all defined Quality Control (QC) procedures, engineering procedures, and specific provisions of the package design approval are satisfied. The QA. program will emphasize control of the characteristics of the package which are critical to safety.
The Radiographic
's Safety Officer shall assure that all radioactive material shipping packages are designed and manufactured under a QA Program approved by the Nuclear Regulatory Commissien for all packages designed or fabricated after the i'
effective date of the QA program. This requirement can be satisfied by l
receiving a certification to this,effect from the manufacturer.

Document Control. All documents related to a specific shipping package 1
will be controlled through the use of written procedures. All document

changes will be performed according to written procedures approved by the Radiographic Safety Officer. The Radiographic Safety Officer shall insure that all QA functions are conducted in accordance with the latest applicable

changes to these documents.

Handling, Storage, and Shipping. Written safety procedures concerning

the handling, storage, and shipping of packages for certain special form 1'
radioactive material will be in accordance with references (a) and (b).
Shipments will not be made unless all tests, certifications, acceptances,

and final inspections have been completed. Work instructions will be

provided for handling, storage, and shipping operations.
Radiography personnel shall perform the critical handling, storage, and shipping operations.

6 Inspection, Test, and Operating Status.
Inspection, test and operat-l ing status of packages for certain special form radioactive material will i
be indicated and centrolled in accordance with references (a) and (b).
Status will be indicated by tag, label, marking or log entry.
Status of i

procedures. Radiography personnel shall perform the regulatory required i
inspections and tests in accordance with references (a) and (b).
The Radiographic Safety Officer shall ensure that these functions are perfor-

Quality Assurance Records. Records of package approvals (including references and drawings), procurement, inspections, tests, operating logs, audit results, personnel training and qualifications and records of l,
shipments will be maintained by the Senior Radiographer.

will be identifiable and retrievable. A list of these records, with their j
j storage locations, will be maintained by the Radiograph; Safety Officer.

Audits.
Scheduled audits will be. performed using written check lists and the results reported to the Commanding Officer. Audit reports will be evaluated and deficient areas corrected.
The audits will be dependent on the safety significance and held at least once a year.
Audit reports will be maintained as part of the QA records.
Members of.the audit team will be from a command other than SIMA florfolk.
