Enclosed is the follow-up report to a reportable occurrence identified at the Memphis State University AGN-201 Reactor Facility on November 16, 1979. The report is submitted in accordance with section 6.9.2 of Appendix A to the Facility Operating License R-127, Docket 50-538.
The reactor was being prepared for startup when a faulty micro- 8%
switch was discovered in the Shield Water Level Safety Channel during performance of the Prestartup Checkoff Procedure. Initial notification was made to your office by telephone and confirmed by mailgram on November 16, 1979.
The faulty switch has been replaced and tested, and normal reactor operations were resumed November 16, 1979.
