Telephone Inquiry on Termination of Activities This refers to a telephone inquiry by Ms. M. Campbell of this office with your-self on October 18, 1979.

This inquiry was an examination activities authorized by License flo. 37-12884-02, as they relate to the termination of activities involving licensed materials.

From this discussion, it is our understanding thay you have disposed of all licensed materials and that you plan to terminate your license by submitting the enclosed Certificate of Disposition of Materials with a letter requesting termi-nation to the USNRC, Division of Fuel Cycle and Materials Safety Radioisotope Licensing Branch, Washington, D.C. 20555.

If our understandings are incorrect, please inform us in writing.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2, Title,10, Code of Federal Regulations, a copy of this letter will be placed in the Public Document Room.
