Appendix A NOTJCE_OF VIOLATION Northern Illinois License No. 12-10513-03 University License No. 12-10513-04 Based on the inspection conducted on October 30 and 31, 1979, it appears that certain of your activities were in noncompliance with NRC require-ments, as noted below.
Items 1 and 2 are infractions, and item 3 is a deficiency.
License No. 12-10513-04 1.
License Condition 12 requires that your sealed sources be leak tested at intervals not to exceed six months.
Contrary to the above, from statements of licensee representatives and the absence of records, no leak test has been performed on your 400 curie Cs-137 Gammator-50 irradiator source since at least 1977. The only record available indicating a leak test was performed is dated 1971, when you received the source.

License Condition 12 requires that you possess and use licensed material in accordance with statements, representations, and pro-cedures contained in application dated December 11, 1967; the University's " Radiation Safety Handbook", dated June 1, 1967; letter dated February 27, 1968, signed by Robert A. Esterlund; application received March 13, 1973; letter dated May 15, 1973; and letter dated October 24, 1977. The above referenced application states that your Radiation Safety Officer will perform GM area surveys of labs monthly, and that each authorized user will survey his lab on a day-of-use basis.
Contrary to the above, since October 1976, labs involved in radio-active material use have not been surveyed as referenced above.
Some labs were surveyed only monthly, and individual users stated that no surveys were performed of labs on days when radioactive materials were used.

License Condition 16.B states that records of leak test results shall be kept in units of microcuries and maintained for inspection by the Commission.
Contrary to the above, since October 1976, no records of leak test results have been maintained for your Mossbauer sealed sources.
