Three Mile Island ?Iuclear Station Unit 1 Docket I!o. 50-289 This letter is written i.n response to the Cc==ission's letter of January 15, 197h which requested that we provide a written explanation of the findings which resulted frca the field inspection conducted on December 26 and 27, 1973 at -the Three Mile Island Nuclear Statien Unit 1.
Our investiga-tien of the findings revealed that all engineered safeguard circuits, in the cable trays audited, have net the routing design criteria.
The attachment contains a su==ary of cur understanding of ycur findings, the corrective action which has been taken to eliminata ncn-safeguard circuit discrepancies and ccrrective steps to avoid farther differe.ces.

The rcuting of circuit EA-310 did nct enter cable tray 2h7. A revised pull slip for EA-310 has been issued indicating "as installed routir.g".
- f. Investigation of QC inspection reports on conduits ME-c3 and CC-33h indicated that tagging has been accomplished on all safeguard circuits after the cables had been routed.
Corrective Stecs to Arcid 7urther Discrerancies An independent audit was performed by the GFUSC Tack Force. As a result of this audit, the following steps were taken:
'a. The tray leading summary has been updated t'c incorporate all kncvn revisions to the cable tray routing.
- b. All documents used to implement design changes were reviewed and all items that affected cable rcuting have been incorpcrated in the data base for the tray leading su=ary.
Cable tray audit instructions have been develcped to assist in obtaining c.
accurate cable counts at coordinate points in a cable tra.v.
Dicererancies have occurred.during cable tray audits as a result of inaccurate cable counts.
These instructions will minimise the occurrence of such anctalies.
GFUSC site QA will continue surveillance by auditing additional cable trays. Ccatinual updating of the tray loading summary and conduit drawings vill be made as "as built and as installed" information is supplied.

In the actuation cabinet rcca, 3h8 ft. elevation, cable QC-35h ws installed in tray 1h3, but this cable was not shcvn en Craving 179, Reiision kl.
- d. In the came location as above, conduit YE-63 contained cables ME-63, "E ~'.
and ME-;3, and were not shown on the signed-off cable pulling slips shavn to the inspector.
Note:
the licensee later produced other copies of the pulling slips in which these cablec were shown to be in conduit ME-65.
- e. In the control building, 333 ft. elevaticn, red safeguard switchgear roc,
there were 60 cables in cable tray 2h7 The tray loading summary showed 60 cables installed.
- f. The cable identification tags were micsing cr illegib'e in numerous locatienc.
For ex1=ple, at one end only of conduits ME-68 and CR-33h, there were 5 unlabeled cables and one illegible label.

- a. An abandoned ca'le was rencved frcm cable tray 83k and revised pull slips for non-safeguard circuits AE-33, AE-35, AE-37 and AE-39 have been issued indicating "ac installed rcuting"
- b. The field inctalled cable trey T-kh-3, in the battery roc elevation 322 ft.,
contained 5 cables whereas the conduit drawing showed a total cf 6 cables.
