Three Mile Island IIuclear Station Unit IIo. 1 Dceket No. 50-289 Inspection 50-289/73-25 Gentlemen:
Your letter of January 17, 1974 stated that during an inspection conducted by Mr. R. H. Erickley of your staff en December 12 - 14, 1973, it was found that certain activities appeared to be in violation of Part 50 of the Commissions Regulations.
The violations and our responses are as stated below:
VIOLATICII:
This violation is considered to be of Category II severity:
Title 10, Code of Federal Regulations, Part 50, Appendix B, Criterict 7, states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accc=plished in accordance with these instructions, procedures, or drawings including changes thereto, which prescribe all activities affecting quality."
Contrary to the above, Initial heling Procedures (IFP) and Initial Fueling Instructions (IFI) were issued and changed in the field in non-ccapliance with IFI-201, Document Cent rol.

Mr. E. J. Brunner, Chie February 12, 197h Reactor Operations Branch REPLY-The evolution performed could have been conducted in accordance with presiously approved construction procedures using United Engineers and Constructicn staff personnel.
We elected not to do this because the necessity to remove and replace the reactor vessel head provided an opportunity to (1) qualify and de-bug the procedures to be used for initial fueling, and (2) qualify and train the personnel to be used during initial fueling operations.
CORRECTIVE ACTION:
As a recult of the experience gained during the reactor vessel head removal and reinstallation operation and of the AEC co==ents, the following action has been taken :
1.
Changes to procedures reflecting the lessens learned have been made.
2.
The document control procedure, IFP-201', has been ec=pletely rewritten to allow a more realistic control.
3 Personnel involved with the initial fueling effort have been briefed and reindoctrinated in the requirements of procedure compliance.
VIOLATION:
This violation is eonsidered to be of Category III severity:
Title 10, Code of Federal Regulations, Part 50, Appendix 3, Criterien XVII, states in part, "... Inspection and test records shall, as a minimum, identify the inspector or data recorder..."
Contrary to the above, the inspector or data recorder was not identified on Form IFP-3033, CRDM Uncoupling Data Sheet.
REPLY:
All initial fueling procedures have been reviewed and revised, where necessary, to ensure that data recorders are identified.
In addition, all perscnnel involved with initial fueling ' cave been briefed in the requirements to identify inspectors or data rec: rders when recording required information.
Ccepliance with the requirements set forth in the initial fueling procedures has been in force since January 10,197h.
