This ec=munication is for the express purpcae of notifying you of a condition existing at Three Mile I: land, Unit 1, which may poccibly be considered reportable in accordance with 10CFR50.55(e)(1)(1), " Conditions of Construction Permits." This condition rela *es to an apparent breakdown in the quality accurance program with regard to inspection and tecting of protective electrical equipment and circuitry. Region 1 AEC/DRO principsl inspector was notified on July 23, 1973.
OE2CRIPTION OF INCIDENT A recent review of the work being performed by Multi-Anp Engineer-ing Services, in support of the Startup and Test program, revealel 1sek of quality control surveillance coverage in areas which we now feel should have had such coverage.
Inspection, setting and testing of prctective electrics 1 equipment la cenlucted by Multi-Amp in accordance with prccedurec approved by Metropolitan Edicon Relay Department.
The initial phase of work accomplished by 4alti-Amp was given quali+y control curveillan'e coversge; however, en April 18, 1972, this sur-veillance was discontinued. This judgment was basec. cn the Icgic that the cetting and testing of protective electrical
- uipmunt w1s a functional test performed and documented in accoriance with Multi-Anp precedures.
In addi-tion, the initial quality control coversge demonstrated that Multi-Amp per-formance was satisfactory.
During the performance of inspecticn and *.est, Multi-Amp 13 re-quired to re=cve and reterminate wiring or replace a =p nents auch as relays or circuit breaker overleada. Carrent accesament cf the critical nature :

In view of the dictstec of this current accecament, quality control curveillance of Multi-Amp hsc been reinstituted. Multi-Amp has been inctructed to notify quality control, in writing, prior to performing cervices n any nu-clear related protective electrical equipment or cir:uita.
In addition, tho fcllowing corrective measurec will be effected:
- 1. Quality Ccntrol perconnel vill incpect and verify a representative 01mple of that Multi-Amp work which was performed without quality control cur-veillance coversge.

tion and/or retect of selectei component:

- 2. All documentation generated by Multi-Amp during the period of time that quality control curveil-lance coverage was not in effect will be reviewed, 100 percent, by a quality accurance engineer dicci-plined in the electrical field. This includes documentation currently available on site and that documentation filed with the Metropolitan Elicon System Relsy Department at Resding, Fennsyl7ania.
