Procedures for Handling TMI Transcript Files i

Attached is a copy or our site procedures for handling transcipts here at TMI.

My purpose in sending them to you is to document that three copies are to be sent by Region I to Trailer 203 at the TMI site, and cne additional copy along with the corresponding tape is to be sent to me at HQ (0IA,1200 Landow,USNRC).

The procedure with respect to stamping DRAFT and the " caveat" (instruction #2) was written only because some of the copies were ccming back to us with no stamp at all.

As scan as you are able to obtain the " caveat" stamp, we would like for all copies to be marked DRAFT and the " Caveat" on all pages by the region.

PROCEDURES FOR HANDLING TRANSCRIPT FILES 1.

Three copies of each transcript will be sent from Region I to Trailer 203 at the TMI Site.

One additional copy along with a copy of the corresponding cassette tape will be sent by Region I to OIA, HQ, NRC, where clerical editing will take place.

The three transcript copies which come to TMI will be filed by TMI number in Trailer 203.

Before filing, however, the copies should be marked (in red for visibility; use stamp if available) in the upper right hand corner as follows:

File Copy and Interviewee Copy - on all pages Review Copy - on first and last pages calyat a minimum 3.

Record the date the transcript is received in the black 3-ring binder, entitled TRANSCRIPT LOG.

If someone wants to check out a transcript take only the REVIEW COPY.

The INTERVIEWEE COPY is to be removed only to give to the interviewee (see instruction 8).

The FILE COPY is to be retained in the file, unmarked (other than to indicate FILE COPY) and removed only for l

xeroxing in the event a second review copy is required. DO NOT TAKE THE i

ENTIRE FILE.

To check cut a transcript, take the REVIEW COPY and put your name and the date on the pink charge out card attached to the file.

When ycu are finished with the transcript, return it to the file and cross your name off the charge out card.

The clerical review is being performed by 0IA at HQ.

When completed, the transcript will be returned to TMI Trailer 203 for a technical review.

If an interview consists of more than one transcript (i.e., TMI Number),

all the transcripts (i.e., TMI Numbers) that constitute a single interview will be sent together in order to be reviewed and typed as a single docu-

(A preprinted form has been prepared instructing the typist to type the individual transcripts as one document.

This form will be completed by OIA at HQ.)

Once the TMI technical review is completed, the t anscript will be sent to Region I to be typed as a final document.

Record the date the transcript is returned to Region I for retyping in the TRANSCRIPT LOG (black binder).

Make sure all TMI numbers are recorded for interviews with multiple transcripts.

In addition, record all TMI numbers shipped each day on the preprinted receipt form (copy attached) and ask the ccurier to sign the receipt for control purposes.

These receipts are to be filed daily in the brown folder labeled " RECEIPTS FOR DRAFT TRANSCRIPTS SENT TO REGICN I FOR RETYPING."

If the interviewee requests a copy of his transcript, pull the INTER'/IEWEE COPY.

Before releasing it, make sure it has been properly stamped, as indicated in instruction 2.

Ask the interviewee to sign and date a receipt form (see blank copy attached).

Circle the word TRANSCRIPTION on line one and fill in the TMI number (s).

Put this form in tne file.

Record the date the transcript is given in the TRANSCRIPT LOG (black binder).
