Dear Mr. Grier:
This letter forwards Licensee Event Report 79-12/lP, required to be submitted pursuant to the requirements of Millstone Unit 2 Appendix A Technical Specifications, Section 6.9.1.8.b.
While at 10% thermal power during the initial power ascension following a shutdown for refueling, two of the four linear power range channels were noted to be inoperable. The power ascension was halted and channel D was placed in the " bypass" condition, channel A was placed in the " tripped" condition and an investigation into the causes of the malfunctions was conducted.
Open connections were found between the channel A and D detector cables and the field wiring due to the pins being backed out at the connectors.
The connections were repaired and the channels returned to service.
Any further actions as a result of subsequent investigation will be addressed'in the two week report.
Yours truly,
~
