In Part 2, indicate by title who is resporrible to assure that all radioactive material shipping packages are designed and manufactured under a QA Program approved by the Nuclear Regulatory Commission for all packages designed or fabricated after effective date of the QA program. Note that this requirement can be satis-fied by receiving a certification to this effect from the manufacturer.

In Part 4, indicate by title what personnel perform the critical handling, storage, and shipping operations.
