Lakeside Bridge & Steel Company Docket No. 99900342/78-02 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on December 11-15, 1978, it appeared that certain of your acitivties were not conducted in full compliance with NRC requirements as indicated below:
Criterion IX of Appendix B to 10 CFR 50 states in part, " Measures shall be established to assure that special processes, including welding,...
are controlled and accomolished by qualified personnel using qualified procedures in accordance with applicable codes, standards, specifica-tions, criteria, and other special requirements."
ASME Code Section IX, Summer - 1977 Addenda, Article QW-201.2 states in part, "The specific facts... and the essential variables...
used in qualifying a WPS (Weld Procedure Specification) shall be recorded in a form called Procedura Qualification Record (PQR)...
The WPS identification (including date and revision number) shall be listed on the PQR."
Contrary to the above, revision numbers and dates of certain WPSs, observed being used during in-process welding, were not listed on the applicable PQRs. The examples are as follows:
revision 1 of WPS 30-B-U3-GF dated June 8, 1978, and PQR 409 dated April 21, 1978, and revision 2 of WPS 49-B-U3 b dated July 17, 1978, and PQRs 402, 407, and 422 dated April 26, 1973, April 21, 1978, and July 22, 1978, respectively.
