O-APPENDIX A NOTICE OF VIOLATION Baltimore Gas cnd Electric Company Decket Nos. 50-317 50-318 Based on the results of an NRC inspection conducted on December 12-15, 1978, it appears that certain of your activities were not conducted in full compliance with the conditions of your NRC Facility License Nos.
DPR-53 and DPR-69 as indicated below.
Item A is categorized as and Infraction and Item B is categorized as a Deficiency.
Unit 1 A.
Technical Specification 3.0.4 states, in part, " Entry into an Operational Mode or other specified applicability condition shall not be made unless the conditions of the Limiting Condition for Operation are met without reliance on provisions contained in the action statements unless otherwise excepted."
Contrary to the above, on December 13,1978, at 5:52 a.m., the reactor was made critical and entered mode 1, with action state-ments of Technical Specifications 3.7.3.1 and 3.7.5.1 in effect.
Conditions for satisfying Limiting Conditions for Operation were not met prior to entry into an Operational mode.
Units 1 and 2 B.
Technical Specification 6.8.1 requires in part, " Written procedures shall be established, implemented, and maintained covering... the applicable procedures recommended in Appendix 'A' of Regulatory Guide 1.33, November 1972."
... Regulatory Guide 1.33, November 1972 endorses ANSI N18.7-1972... ANSI N18.7-1972 section 5.3.5, Maintenance Procedures, states, " Instructions shall be included, or referenced, for returning the equipment to its normal operating status. Operating personnel shall... document its functional acceptability."
Calvert Cliffs Instruction (CCI)-200B, Maintenance Requests, August 31, 1977 implements these requirements of ANSI N18.7-1972, and further states, "The foreman (maintenance) must determine the post maintenance test requirements associated with the maintenance."

Contrary to the above, as of December 15, 1978:
Documentation of equipment functional acceptability did not exist in 10 of 36 maintenance requests reviewed.
In one instance, for the maintenance on No.11 Control Room Air Conditioning Unit, the maintenance foreman's instructions required a one hour trial run prior to return of equipment to service.
No record to substantiate performance of the functional test could be located.
Instructions for equipment return to normal operating status were not included or referenced, in 12 of 36 maintenance request reviewed.
